They are seeking an experienced Support Coordinator to join their close-knit team and play a key role in empowering NDIS participants to achieve their goals.

About the Role

As a Support Coordinator, you will manage a caseload of NDIS participants, empowering them to navigate their plans effectively and access the supports they need. Your role is essential in helping clients build capacity, exercise choice and control, and work towards their individual goals.

Key Responsibilities

  • Assisting participants in understanding their NDIS plans and using their funding flexibly.
  • Providing personalised support by identifying needs, linking participants with appropriate services, and ensuring their goals align with the right supports.
  • Collaborating with informal supports, allied health professionals, and government agencies to facilitate comprehensive care.
  • Promoting independence, choice, and control, helping participants build skills and take charge of their lives.
  • Coordinating and liaising with service providers, reviewing participants' progress, and setting new goals where needed.
  • Organising and leading care team meetings to ensure participants receive well-rounded and effective support.
  • Preparing reports for NDIS plan reviews to advocate for participants' needs and ongoing support.
  • Working alongside other service providers to strengthen participants' abilities to achieve long-term success.

Skills and Experience:

  • Driver's licence
  • Strong administration skills
  • Minimum of 12 months experience
  • Strong can-do attitude and initiative
  • NDIS Worker screener check
  • Excellent problem solving and multi-tasking capability
  • Higher education within the mental health/disability seminars would be admirable
